Required Documents

Required for Returning CGHA players
1 - completed registration form and cheque

New players (i.e. not previously registered with any Girls Hockey Association)
1 - Completed registration form and cheque
2 - Proof of age (copy of birth certificate)

Players previously registered with another Girls Hockey Association
1 - Completed registration form and cheque
2 - Proof of age (copy of birth certificate)
3 - Permission to Skate and/or Release form (Must have before going on ice) Click For Forms

Fees 20010/11 Season

– Cost TBA

– $25.00 less for 2nd, & 3rd child in the same family

– $100.00 lottery fee per family
– separate payment must be included upon registration, if check, dated no later than May 31, 2009. These Tickets Are Non– Refundable Tickets are available now.

– $75.00 deposit per player dated now, remainder postdated for Oct.1 & Nov.1, 2009

LATE FEE: $50.00 per player if you register after May 31/09

Late Fee Does not apply to new players.

*note* Additional Rep Fees will apply for those playing on Clearview Representative (REP) Teams.
